First Visit

Preparing for Your First Visit
We request that all new patients provide us with basic health and general information.

We prefer that you complete the following forms online. If you are not able to do this, please complete and print the PDF forms and mail them to our office prior to your scheduled visit.


Vaccine Record
We will need a copy of your child’s vaccine record. Please send a copy of your child’s vaccine record to our office before your first visit. If this is not possible, please bring the vaccine record to your visit.


Office Protocols

All patients must have a scheduled appointment. Please use our Request an Appointment online form to set a date and time that is convenient for you.

No Show Policy

We expect our families to cancel any appointment that they will not be able to honor. If your child misses an appointment without a call to cancel or reschedule, we consider this a “no show”. We maintain a No Show Policy where any child that has three “No Show” appointments will be considered for dismissal. 

Late Policy

When a patient is more than 5 minutes late, the appointment will be rescheduled. If you are running late for your child’s appointment, please call us so we can make accommodations to have your child seen. We will either reschedule for a later time or have your child see another provider. When our patients are on time, we see all patients in a timely manner!



Office Visits

Sick Visits
If your child is sick, the providers will want to see him or her that day. Please call the office to make a same-day appointment.

Well-Child Visits
Dr. Brown and her nurse practitioners will see your child for the following well-child visits:

  • 3-5 day visit
  • 2 week visit
  • 1 month visit
  • 2 month visit
  • 4 month visit
  • 6 month visit
  • 9 month visit
  • 12 month visit
  • 15 month visit
  • 18 month visit
  • 24 month visit
 After the first two years of life, the providers will see your child once a year. They recommend that children be seen yearly until they are 18 years of age.

You can make well-child appointments online. If you are not able to do this, you may call the office to make the appointment.

Cancellations

Please be respectful and courteous to the other families who come to see us! If you are not going to be able to keep your appointment, please call our office at 972-875-8300 as soon as possible to let us know.
